New Immunization requirements
New Immunization requirements for students entering 11th Grade for the 2022-2023 school year
All students attending in building, virtual or transferring into the 11th grade
are required by law to show proof of
one booster dose of the meningococcal conjugate (MCV4) vaccination.
If the first dose was received on or after their 16th birthday a booster is not required.
Please provide Nurse Smallwood with a new Georgia Immunization 3231 form which can be obtained by contacting your child’s Primary Care Provider or
Health Department.
